Loss or damage of your ID card abroad should be reported immediately to the Polish consular office.

Report the loss or damage of the ID card in person at the consular office or in writing by mail or fax.

You don’t need to make an appointment.

  • A completed form for reporting the loss or damage of the ID card – you can download the form here or ask for it at the consulate,
  • ID card – if you report damage to the ID card,
  • other identity document, for example passport – if you have one.

The application itself is free of charge.

The consul will accept your report about the loss or damage of the document immediately.

If you report the loss or damage of your ID card in person at the consular office – you will receive a certificate confirming this fact.

If you are sending the form by post or fax – be sure to indicate on the form that you want to receive a certificate of loss or damage to the ID, for example by post.

Keep the certificate – you may need it. It is valid until a new ID card is issued, but not longer than 2 months.

Remember that the certificate you will receive at the consulate is not an identity document.

Your ID card will be invalidated on the day you report its loss or damage to the consul.

Can I submit an application for an ID card at a consular office?

The consul does not issue ID cards. An application for an ID card can only be submitted in Poland, at any commune office.

I lost my passport and ID card and I have to return to Poland. Can the consul issue me a document authorizing me to cross the border?
